Clear Floor Space Drives the Plan

The defining difference between an accessible bathroom and a conventional one is how much of the floor has to stay empty. A turning space for a wheelchair is a substantial circle or T-shape, and it cannot overlap with fixtures.
That single requirement usually makes an accessible bathroom larger than a standard one, and it explains why converting a small existing bathroom rarely works well: the fixtures fit, and the empty space does not.
Transfer space is the second demand. A person transferring sideways from a chair to the toilet needs the approach side left clear, which fixes which side of the toilet can have a wall and which cannot. Deciding this early determines the whole layout; deciding it late means moving the waste.
Draw the clear zones first and place fixtures in what remains, exactly as with a small bathroom but with larger and less negotiable zones.

Level Access and Showering

A threshold is a barrier, so accessible showers are built level with the floor. In practice that means a wet room: the floor is tanked, falls to a drain, and there is no tray upstand to cross.
A linear drain suits this particularly well, because the single-plane fall it allows is easier to build accurately across a larger area and easier to keep shallow enough to wheel over.
A shower seat, either fixed or fold-down, and the grab rails around it are positioned relative to the transfer approach rather than to the shower fittings. Both must fix to structure, which means backing installed before boarding.
For the enclosure itself, a fixed screen with a return panel is usually the answer: it controls spray without introducing a door, a threshold or a swing arc. Where a door is unavoidable it should open outward, and its arc must be clear of the turning space.
Controls should be reachable from both standing and seated positions, and positioned so the shower can be turned on without entering the spray.

The Basin and the Space Beneath It

A basin that cannot be approached in a chair is not usable, and this is where conventional vanities fail: a cabinet with drawers to the floor leaves nowhere for knees.
The requirement is clear knee and toe space beneath the basin, which means either a wall-hung basin with no cabinet, or a vanity designed with an open recess under the bowl. Wall-hung is the more common answer because the mounting height can also be set to suit the user.
Where a vanity is wanted for storage, the practical arrangement is drawers to one side and open knee space under the bowl. That gives usable storage without blocking the approach.
Exposed pipework under an accessible basin should be insulated or shielded, because a person seated close to it cannot easily move away from a hot surface. And the basin depth should be modest, so the tap is within reach from seated.

Fittings, Controls and Details

Beyond the layout, a set of component choices makes the difference between technically compliant and genuinely usable.
Lever or single-handle taps rather than rotating knobs, because they can be operated with a closed hand or a forearm. Thermostatic control on the shower, because a person who cannot move quickly out of the spray is at greater risk from a sudden temperature change.
Contrast matters more than it seems. Fittings, rails and seats that contrast visually with the wall and floor are considerably easier to locate for anyone with reduced vision, and it costs nothing to specify.
Grab rails should be specified as structural components with a stated load rating, and fixed to backing rather than to plasterboard. A rail that pulls out of a wall is worse than no rail, because it will be trusted.
Finally, avoid slippery floor finishes and check the slip rating of the tile, particularly in a wet room where the whole floor gets wet.
